About Your Credit Report and Your Credit Performance
You agree that we have the right to obtain a credit report in connection with our review of your application and, subsequently, in connection with a requested charge to any account with us, the renewal of your account, or a change in your credit line; and that we have the right to report to others our credit experience with you. Upon your request, we will tell you the name and address of each consumer reporting agency from which we obtained a consumer report about you. After your account is opened we will periodically review your credit performance. If you do not maintain your credit performance, we may change your account terms.
Your Telephone Number
By providing us with a telephone number(s), at the time you apply or anytime thereafter, you authorize Farm Bureau Bank and its affiliates, agents and independent contractors, to contact you regarding your account at such number using any means of communication, including, but not limited to calls placed to your cell, mobile or other phone using an automated dialing service, pre-recorded message or text messages. Balance Transfers. Here's How They Work.
To request a balance transfer, please enter the 15/16 digit account number from the MasterCard, Visa, American Express, or Discover Card account that you want us to process a balance transfer to. If the total amount you request exceeds the assigned credit line on your new account, we may either decline the request or send a partial payment to your creditor(s). The amount of available credit on your new account will be reduced by the amount transferred and the related balance transfer fees. We reserve the right to decline to process any requested balance transfer. We will not process a request to transfer a balance from any other account you may have with us. Balance transfers incur interest from the transaction date. You should not transfer the amount of any disputed purchase or other charge. If you do, you may lose your dispute rights. Upon approval of your application we will process the requested balance transfer payment to the account(s) which you have requested on the 10th day after we've mailed you your card; during this 10-day time period you may cancel your balance transfer by contacting us at the number on the back of your card.
A balance transfer may take up to four (4) weeks to post to your other credit card accounts. Continue to pay each creditor until the balance transfer appears on the other account as a credit. If you would like to complete a balance transfer from another account that is not a Visa, MasterCard, American Express, or Discover Card, please call the phone number on the back of your new credit card once you are approved. Balance transfers are subject to availability.
Cash Equivalent Purchases
The purchase of money orders, traveler checks, foreign currency, lottery tickets, gambling chips, wire transfers and other similar transactions are treated as cash advances. Please see the Cardholder Agreement for more details.

Your Request for Credit
You are submitting this application on behalf of the Company as its authorized representative and on behalf of yourself as an individual guarantor of payment. You certify that you are an owner, officer or partner of the Company with the authority to bind the Company to the terms of the Farm Bureau Bank Premier Business Visa® Card Agreement which will be sent to you with your Card. You also, in your individual capacity (even though you may place a title or other designation next to your signature) unconditionally guarantee and promise to pay all indebtedness of the applicant at any time arising under or relating to this application as well as any extensions, increases, or renewals of that indebtedness.
You understand that we will not return this Application. You understand that you must provide all the information requested in this Premier Business Visa application, any required financial statements or other information as may be necessary to process your application and certify that such information is accurate to the best of your knowledge as of the date provided. You authorize us to verify the information on the application and additional information and exchange information about you and the Company including requesting reports from credit reporting agencies for the sole purpose of an update, extension of credit, review or collection of this account. Upon your request, the Bank will inform you of the name and address of each consumer reporting agency from which it obtained a consumer credit report. You authorize us to verify employment history, and to share any information we obtain about you with third parties including our affiliates.
Farm Bureau Bank will notify you if more information is needed to process your application. We may send the request for additional information by e-mail or facsimile unless you indicate that you request us to send such communication to you only by mail.
Balance Transfer Information
Balance transfer information must be completed in full in order for your balance transfer request(s) to be processed. Balances to be transferred will be processed in the order listed, subject to your assigned credit limit. You must continue to make any payments required by the terms of the account from which you are transferring a balance. When you transfer balances using this form or transfer a balance over the phone, the amount transferred will be treated as a purchase; however, finance charges accrue from the transaction date. If the amount you transfer would cause the balance to exceed your credit limit, then only the portion of the transfer that would bring your balance up to your credit limit will be transferred.
Changes to Account and Benefits Terms
We reserve the right to change the interest rate and other account terms in accordance with the Line of Credit and Visa Business Card Agreement and Nevada law. We also reserve the right to change the benefits and features associated with the card or the account.

How We Will Calculate Your Balance:
We will use a method called "daily balance (including new purchases)". If you are a resident of Iowa at the time the account is opened, we use a method called "average daily balance (including new purchases)". These methods are explained in the cardholder agreement.
Loss of Introductory APR:
We may end your introductory APR and apply the Penalty APR if you make a late payment.
Military Lending Act: Covered borrowers under Military Lending Act; Federal law provides important protections to members of the Armed Forces and their dependents relating to extensions of consumer credit. In general, the cost of consumer credit to a member of the Armed Forces and his or her dependent may not exceed an annual percentage rate of 36 percent. This rate must include, as applicable to the credit transaction or account: The costs associated with credit insurance premiums; fees for ancillary products sold in connection with the credit transaction; any application fee charged (other than certain application fees for specified credit transactions or accounts); and any participation fee charged (other than certain participation fees for a credit card account). The Farm Bureau Bank Premier Business Visa® Card is issued by Farm Bureau Bank, FSB pursuant to the Premier Business Visa Card Agreement that will be included with your Card. The account is designed exclusively for business, commercial, agricultural or organizational purposes and is not for personal, family or household use.
Summary of Selected Account Terms:
Index: "Index" means the "Prime Rate" used in determining the Annual Percentage Rates (APRs) on your account each billing cycle and is the highest rate published in the Money Rates column of the Wall Street Journal on either the first or last day of each billing cycle. The Prime Rate is 5.25% as of August 31, 2019.
Margin: "Margin" means the interest rate differential between the Index Rate and the APR. The Margin is expressed as a number of percentage points.
Finance Charges on Purchases: : "Finance Charges" accrue on Purchases from the date the Purchase is posted to your Account. You may avoid paying Finance Charges on the Purchase portion of the "New Balance" shown on your monthly statement if the New Balance is paid in full by the "Payment Due Date" shown on the monthly statement. The Payment Due Date will be at least 22 days later than the "Closing Date" shown on your monthly statement. If you do not make full payment by the Payment Due Date, you will be charged a Finance Charge computed on the "Average Daily Purchase Balance" for the current billing period.
Finance Charges on Cash Advances: "Finance Charges" accrue on Cash Advances from the day you receive a Cash Advance until the Cash Advance is paid in full.
Minimum Finance Charge: If your monthly statement shows an amount subject to Finance Charges, the Finance Charge will not be less than $0.50.
Introductory APR for Purchases (New Accounts Only): For the first three billing cycles from the account open date, the Introductory APR for Purchases is a fixed 0.00%.
Variable Rate Plan for Purchases: : The variable APR for Purchases will be 9.24% to 16.24% based on your creditworthiness. The APR will vary with the market based on the Prime Rate. The Variable APR for Purchases will not be lower than 6.99% APR.
Promotional APR for Balance Transfers: For the first three billing cycles from the account open date there is no balance transfer fee and the Promotional APR for Balance Transfers is a fixed 0.00%. In order to receive this Promotional APR, you must transfer your balances by calling us at 1-800-492-3276. When the Promotional period ends, the Standard Variable APR for Purchases applies.
APR for Balance Transfers: After the Promotional period expires, the Variable APR will be the same as the Standard Variable APR for Purchases.
Variable Rate Plan for Cash Advances: The Variable APR for Cash Advance balances will be 9.24% to 16.24% based on your creditworthiness. The APR will vary with the market based on the Prime Rate. The Variable APR for Cash Advances will not be lower than 6.99% APR.
Default APR: If we do not receive a payment equal to or greater than the "Minimum Payment Due" within 22 days following the "Payment Due Date" on any monthly statement, the APR for all balances on your account will increase to a Variable APR of 25.24%. The APR will vary with the market based on the Prime Rate.
Line Access Checks: Line Access Checks will be treated as Cash for the purposes of calculating finance charges. No cash advance fee will be charged on the use of Line Access Checks.
Annual Fee: There is no Annual Fee for this Account.
Cash Advance Fee: You will be charged a 3.00% Cash Advance Fee, minimum $10, when you use your card to access cash on your account from a Bank, automated cash machine, or other places where cash may be obtained through the use of the card.
Late Payment Fee: If your payment is not made by its due date, a Late Fee of $39 will be assessed.
Over-the-Limit Fee: If the balance on your monthly statement exceeds the Credit Limit for the Account on the Closing Date of the monthly periodic statement, an Over-the-Limit Fee of $39 will be assessed.
Returned Check Fee: A Returned Check Fee of $29 will be assessed for each return by a bank or other depository institution of a dishonored check, negotiable order of withdrawal, or share draft issued by you to us in connection with the account.
Returned Convenience Check Fee: A Returned Convenience Check Fee of $29 will be assessed for any convenience check returned by us.
Other: Your initial order of checks is free. A fee of $4.95 will be assessed for each check re-order you request. A charge of $5.00 will be assessed for each replacement plastic. Rates, Indexes, Margins, Fees, and other terms described here are typical of those offered and in effect on Farm Bureau Bank Premier Business Visa credit card accounts and are expected to be applicable to your account.
